Refunds. It is not customary for Practice to issue refunds. If Practice, in its sole discretion,
elects to offer a refund, such amount will be reconciled against amounts paid minus the actual
value of professional services rendered to you while the Agreement was in effect. 

Returns. If the product is unopened and untampered with, then a full refund may be issued minus a 10% restocking fee. 

Cancellations. If you need to cancel an appointment for any reason, we request that you
provide us with a minimum of 24 hours’ notice (excluding same day or next day
appointments) if you are unable to attend a scheduled appointment. Your failure to
provide us with such notice may result in a cancellation fee of $50. Consistent
cancellations without notice or chronic lateness may result in the termination of this
Agreement.
